If you are a new patient to our practice, if you have not seen us in three years or more or you have changes to your information, then click on the form tab above, select new patient forms, print out all three of them, fill them out and bring them with you to your appointment.

Cancellation Policy
Please let us know within 24hrs of your appointment if you need to cancel or reschedule. If this is not done a $20 fee will be applied.

This is due at the time of your visit, if you are unable to pay a $5.oo fee will be added to balance due.​​​

All patients must bring their insurance card(s) with them to the appointment so that we may appropriately bill. Without the insurance information you may be responsible for payment.

Self Pay​       
We are very accommodating to each individual's situation. If you are not able to pay same day we ask that you pay whatever amount you are capable of paying that day, and then from there our billing department will work out with you a payment plan that fits your budget. We understand the financial times we all are in and we do not believe that should decide your medical attention. We do offer a discount if you pay in full same day.

Insurance Referrals
If your insurance requires you to have a referral or you are not sure if you need one, please speak to your Primary Care Provider, they need to issue the referral. If there is not one in place your insurance company may deny your visit and then the balance becomes your responsibility.
