General Information


If you are a new patient to our practice, if you have not seen us in three years or more, or if you have changes to your information, please go to the forms page and print out all three forms for New Patients, fill them out, and bring them with you to your appointment.

Cancellation Policy
Please let us know within 24 hours of your appointment if you need to cancel or reschedule.

Copays are due at the time of your visit.

Important Insurance Information
Please call the office at (207) 351-3525 to find out the most updated list of insurances that we accept.

**We do NOT take York Hospital's Community Care Insurance**

All patients must bring their insurance card(s) with them to their appointment so that we may appropriately bill. Without the insurance information you may be responsible for payment.

Self Pay
We are very accommodating to each individual's situation. If you are not able to pay the same day as your appointment, we ask that you pay whatever you are able, and then from there our billing department will work out a payment plan with you that fits your budget. We understand the financial times we are all in, and we do not believe that should decide whether you receive medical attention. We do offer a discount if you pay in full the same day as your appointment.

Insurance Referrals
If your insurance requires you to have a referral or you are not sure if you need one, please speak to your Primary Care Provider (PCP).  Your PCP needs to issue the referral. If there is not a referral in place at the time of your appointment, your insurance company may deny your visit and the balance will become your responsibility.
